Effective 7/1/2026 plan participants visiting or living outside of Arizona may use the National BlueCard Network.
In-network providers and facilities can be located on the BCBSAZ websitewww.azblue.com. Select “Find Care”, then “Browse the Network”. Select the year, Employer Provided, Medical and then Statewide/Nation PPO.
Mayo Clinic may show up in your search results but continues to be non-network.
Prior to 7/1/2026, plan participants visiting or living outside of Arizona may use the PHCS network. Visit www.multiplan.comand select “Find a provider”. On the next screen select “Select Network”and choose PHCS – I don’t see any of these statements – Back of card. Then you can search for specialties, specific providers or types of providers/facilties.
